Wage rate is $20.21-$29.54 per hour, dependent on experience. Under the direction of the Clinic Manager, Care Coordinator is responsible for the establishment and ongoing management of relationships between the patient and provider teams at Modoc Medical Center Clinic to implement and facilitate a Patient Centered Medical Home (PCHM) model. The model uses a team approach focused on a continuous partnership between patient, caregiver a, physician, hospital, and community resources to provide longitudinal, coordinated care for patients. Must have a high school diploma or GED, current CPR certification, two years professional experience in a healthcare environment preferred and care coordination or case management experience desirable.
