Reports to: Executive Director. Employment Status: Full-Time, Non-Exempt. Starting Salary Range: $18-$20 per hour (DOE). (After completion of a probationary period, an employee qualifies for a fringe benefit package of medical, dental, vision and life insurances, a 401 (k)-retirement package and 13 paid holidays annually. These benefits are equivalent to approximately an additional $10 per hour).
GENERAL RESPONSIBILITIES: This position integrates the responsibilities of a Community Health Representative and Elder Services Coordinator. The purpose is to provide transportation and escort services for clients accessing health services, conduct home visits to support vulnerable populations including elders, and deliver advocacy, education, and case management. The role also aims to reduce elder abuse, support independent living for elders, and connect individuals with available health and social service resources. TYPICAL DUTIES: •Provide safe and timely transportation for clients to and from medical appointments and other essential services. • Conduct home safety assessments and wellness checks for vulnerable clients, including the elderly. • Deliver medications and essential supplies to clients’ homes. •Assist clients in applying for and maintaining third-party eligibility (e.g., Medi-Cal, Medicare, SSI). • Refer findings or concerns requiring follow-up to appropriate medical or social service providers. • Promote nutrition, exercise, and other wellness activities among the client population. • Coordinate and facilitate elder-focused activities and recreation to reduce isolation and increase engagement. • Support efforts to prevent elder abuse through education, monitoring, and timely interventions. • Maintain accurate records of client interactions, travel logs, assessments, and service delivery and input into RPMS. • Advocate on behalf of clients in accessing appropriate services and navigating social systems. • Educate clients and families about health and wellness resources available through the Center. • Represent SFHC positively in the community and collaborate with local health and social service agencies. • Perform additional duties as assigned to meet the evolving needs of clients and the organization.
KNOWLEDGE AND SKILLS REQUIRED: • Knowledge of general health care and social services practices, especially as they relate to Native American communities. • Familiarity with issues affecting the elderly and experience conducting home visits and case assessments. • Ability to recognize health or safety concerns and respond appropriately or refer to professionals. • Excellent communication, advocacy, and interpersonal skills. • Working knowledge of third-party benefits programs and social service systems (Medicare, Medi-Cal, SSI, etc.). • Strong organizational skills and ability to maintain confidential records accurately. • Basic First Aid and CPR certification preferred. • Computer skills, including data entry (e.g., RPMS or similar systems). • Must be dependable, respectful, and able to work independently as well as collaboratively. • Valid California driver’s license/Real ID, good driving record, proof of insurance, and dependable transportation required. • Ability to travel for trainings, workshops, and other work-related duties. DRIVING AND ABILITY TO TRAVEL: Current California Driver’s License / Real ID and insurable per Strong Family Health Center automobile insurance carrier. Employee must be able to travel to workshops and trainings, overnight travel is required. Travel is primarily within California, however, can be nationally.
